If you wish to order the courier delivery of a passport from the Canada Visa Application Center in Warsaw to one of the Nova Post branches in Ukraine, you need to:

1. Make a payment (also possible by cash/card in the Visa Application Center or through a payment gateway as described above) to the Visa Center account in the amount of PLN 157.50 per passport –each passport is sent in a separate envelope:

   • Recipient's name: VFS Global Poland Sp. z o. o.

   • Recipient's address: Aleja Armii Ludowej 26, 00-609 Warszawa

   • Recipient bank’ name: Bank Handlowy w Warszawie S. A.

   • Address of the bank's: ul. Senatorska 16, 00-923 Warsaw

   • Bank BIC/SWIFT code: CITIPLPX

   • Account № IBAN formaе: PL75103015080000000817777007

   • ATTENTION:when making the payment, please make sure that the specified above amount PLN 173.00 is credited to the visa center's account after deduction of all possible indirect fees by the bank/banks making the transfer.

2. Submit the payment confirmation together with the rest documents and passport when submitting them to the Visa Application Center (if the documents are sent to the Visa Center by post/courier - attach the confirmation to the parcel).

3. Attach a separate declaration with all data for sending the passport to one of the Nova Post branches on the territory of Ukraine:

   1. NAME AND SURNAME OF THE RECIPIENTNAME AND/OR NUMBER of the NOVA POST

   2. BRANCH TO WHICH THE SHIPMENT SHOULD BE DELIVERED

   3. The ADDRESS of the NOVA POST BRANCH TO WHICH THE SHIPMENT SHOULD BE DELIVERED

   4. RECIPIENT'S CONTACT E-MAIL

   5. RECIPIENT'S PHONE NUMBER
