Overview

Tourist visa is granted for those where the intention is to visit Czech Republic for tourism and sightseeing without having an invitation from Czech Republic referent.

Visa Fees

Please Note:

There is a service charge of 49 AZN (Inclusive of 18% VAT) levied per application over and above the visa fees

The visa application centre offers an optional service at a nominal additional cost:

All Visa Fee are to be paid in cash only in Euro.

All VFS service charges are to be paid in cash only in AZN.

All optional services are to be paid in cash only in AZN.

Documents Required

Requried documents to get schengen visa in order to tourism or treatment to Czech Republic

Application form

• Must be filled with capital letters only in English, in latin alphabet
• Must be signed by parents/ legal guardian (if applicants under age18)

Photo

• Biometric photo
• Colored, 3,5 x 4,5 (approximately 1,2 x 1,6 inch)
• Light and monophonic background (white)

Passport

• Passport must be issued not late within 10 years
• At least 3 months till intended visit from schengen area
• Minimum two free pages
• Passport copy: identificatin page and all previous schengen visas (with stamps)

Identification card

• Original and copy of two pages

Confirmation of staying and purpose of visit

As a tourist: confirmation from hotel or sanatorium where will be shown

name, surname, date of birth,number of passport, reservation date , signiture and stamp of hotel

For treatmetnt: offical document from medical organization at Czech Republic, confirmation of medical service in this organization. In document should be shown period of staying and approximately value of treatment. In addition treatment doctor must have recomendation letter.

Medical Insurance

• Medical insurance to Schengen area, covering the entire period of intended stay, the minimum coverage must be 30.000 EUR/ 50 000 US
• For registration double visa, must be submitted insurance for both trips
• For multi visa you should sign 4th pages of application form , where customer states to cover insurance the entire period of trip

Proof of applicants regular income

• Reference from work
• For business visa: The company registration in the chamber of commerce and a copy of the bulletin of the trade register (VOEN)
• Reference from Pension fund proof of getting pension
• All references from regular income

Proof of applicant’s pay slip

• Reference from bank ( at least 60 EURO for one day for one person, it’s possible with EURO or different currency )
• Copy of international credit card (credit card) from the same bank which you get reference

Sponsor:

• Notarial confirmation of sponsor letter in english or russian language, with contact details (working cell and home number) of sponsor
• Reference from bank or work sponsor person
• Copy of sponsor ID

If applicant is minor

• Copy of certificate of birth
• Power of attorney from a not-travelling parent(s) or legal guardian(if customers goes alone)

Attention: All documents and copies (except passport copy) in Azerbaijan language must be translated to Russian or English language with notarial confirmation.

Attention: Dear customers! All checklist documents suggested to Azerbaijan citizens. Citizens from different countries can submit all noticed documents only if he/she has registration card to Azerbaijan Republic, after his/her return from schengen area must be minimum 3 months

Attention: The consulate at Czech embassy may need extra documents. All documents must be submitted not earlier 6 months (180 days) applying to schengen area.

• 1.Certificate of birth must be translated into in English or in Russian with notarial affirmation (for everyone under 18 age)
• 2.Transportations between Shengen countries must be shown
• 3.Bank statement and the copy of the plastic card from the same bank
• 4.All the documents except the identity card are accepted in English or Russian
• 5.Application form must be filled with capital letters only in English with Latin graphic
• 6.Confirmation letter from the hotel must be signed and sealed
• 7.It is necessary to show the sealed and signed documents about the places you will stay during your trip.

Processing Time

The processing time of visa applications are in the discretion of the Embassy of Czech Republic in Baku.Visa applications shall be decided on within 15 calendar days after the embassy or consulate has received the application. The decision period may be extended to 45 days when further consideration is needed.

• Extract from Real Estate Cadastre must not be late 6 months

• It must be shown contact details of educational institution at Czech Republic and customer’s data (name, surname, number of passport, date of birth)

Confirmation of accomodation

Confirmation of living from hotel where will be shown:

Required documents to get schengen visa for a member of family and citizen from European Union

Member of family and European Union citizen – he is her spouse, parents, if European Union citizen under 21 age , which he lives together with her, descendant under 21 age, or descendant whose spouse is European Union citizen, at the same time kinship or close relation with European Union citizen. If it is educational trip on that time member of family- it’s only spouse or minor

Vulnerable person - he’s supported by European Union citizen or his spouse, preparing for a future profession or foreign citizen, who for his sickness or trauma can’t prepare his future profession or for his medical disability he can’t work.
